If you are a healthcare provider wishing to refer a patient to Indiana Spine Group, please see the attached Patient Referral Form (PDF) to find the appropriate location and/or physician listed on the form. Please fax the attached form, any patient demographics and previous testing that has been completed.
Most healthcare providers referring patients to us have performed an initial visit, which often includes a physical examination, lab work, X-rays, CT and/or MRI scans, but this is not required. Referring providers will receive a complete report from Indiana Spine Group. Please call our office with any questions at 317.228.7000.
